Each person who will be using this plan will need their own application but you may photocopy the application that we will fax or mail to you. The prices are in US dollars including the shipping which is $9.95 per order. Husbands and wives ordering at the same time will pay only one shipping charge.
Once you have filled out the application including the release and the credit card information then, you will need to fax it directly to 1-877-278-5359, Canada Health Systems's toll-free fax line. ( Remember to sign in all three places). The ideal order is for the 90 day supply with 3 additional re-fills. By law they cannot ship you more than a 90-100 day supply.
The initial order will take approximately 10-12 days depending on weekends and Holidays from the time you fax your order to Canada Health Solutions to the day it arrives in your mail box. Each re-order after that should arrive in just 6-8 days.
